Description

Green Prescription (GRx):

''GRx'' is a referral based health and wellbeing service for adults 18 years and over. It is best suited for adults who would like encouragement and motivation to become active and improve their lifestyle, help with a long term medical condition, or those who may be feeling stressed, depressed or anxious.

The service is delivered through face to face, phone or virtual consultations, group-based community nutrition and wellbeing programmes, online webinars, online live exercise sessions as well as pre and post-natal activity advice and guidance.  

What we offer: 

  • Community, TeleHealth or online support from a Healthy Lifestyle Advisor
  • Regular check ins for up to 3 months 
  • Live Streaming Exercise Classes,
  • Nutrition & Wellbeing Webinars 
  • SMS Motivational Support 

Translation services are available free of cost.

How do I access this service?

Referral

Referral Criteria:

  • 18 years and over
  • Ready to make changes to lifestyle 
  • May have a medical condition as a result of a sedentary lifestyle 
  • May have depression, anxiety or stress  

Primary care based referrers should send eReferrals through HealthLink forms. Using this referral pathway is quick, easy and secure. 

  • CareConnect is the preferred eReferral method for all primary care 

All Health Professionals can refer their patient or client to Green Prescription, or people can also choose to Self Refer themselves by calling 0800 228 483 or online HERE

Active Families programme

The 'Active Families' programme is well suited to children that are inactive, above a healthy weight and would like regular motivation to make changes with their whanau. Active Families is for school aged children aged 5 - 17 years, along with the support from their parents and wider whanau. Service types: Family / whānau support, Wellness / healthy lifestyle, Exercise and activity, Pregnancy and parenting, Postnatal support, Whānau Ora, Kaupapa Māori, Pacific People, Asian services.

PRIMARY CARE eReferrals

eReferrals can be made by Primary Care Health Professionals through HealthLink forms, using Care Connect or Care Select. 

Step 1 - Go to ‘Green Prescription’ in the CareConnect eReferral platform
Step 2 - Please provide a clear reason for the referral and give the patient a Green Prescription flyer
You can advise the patient they will be contacted within 4 working days

DHB / Secondary Care eReferrals

DHB based health professionals can easily send eReferrals through the Regional Clinical Portal / Concerto by using the 'Green Prescription' form.

If an eReferral cannot be made, a referral can be made through the Sport Auckland website HERE
