Auckland DHB Mobility Solutions

Hours

Our reception is attended from 8am to 4.30pm Monday to Friday. When you are working with a therapist they will make appointment times with you. These are likely to be a mix of home, work, school and clinic based appointments.

Description

Our Service


Mobility Solutions is a wheelchair and seating assessment service based at Rehab Plus in Pt Chevalier, Auckland. We hold clinics on site at Rehab Plus and in some community venues. We also visit people in their own homes, schools and work places.

Mobility Solutions provides a service to people whose mobility and seating requirements are long term, complex and require individualised components. We cover the greater Auckland region and do not work with ACC claimants.

There are other services that can meet the needs of clients with less complex requirements; these include:

  • Community Occupational Therapists or Physiotherapists in each District Health Board
  • School Therapists
  • Hospital Therapists
  • Child Development Team Therapists
  • Group Special Education Therapists
  • Therapists who work in other facilities such as rest homes or private hospitals.

These therapists can also utilise the ACCESS clinic run by Accessable if they wish.
 

We do not hold any equipment or budget for equipment. Following your initial assessment we make funding applications to the fund holder Accessable on your behalf. This application is based around the Ministry of Health Funding Criteria. We then coordinate trial or fabrication of the equipment we have recommended and work with you to come to a solution that will best meet your mobility and wheelchair seating needs.

Repairs and Maintenance

The Team


We are a team made up of wheelchair and seating therapists who are either occupational therapists or physiotherapists. We also have a therapy assistant, wheelchair technicians and other support staff to help us in the day-to-day running of our service. The clinical team are supported by a practice supervisor and a team leader.

We would love to hear from enthusiastic NZ registered physiotherapists or occupational therapists who are interested in joining our team.  Please contact our team leader to discuss any possible opportunities.

We also hold clinics in conjunction with specialist services e.g. special schools or multidisciplinary specialist clinics. If you think that your service or facility would benefit from an onsite clinic, please contact our team leader to discuss. 
 

 

Referral Expectations

Make a Referral
 

Mobility Solutions accepts referrals from clients, whanau/family, support people and health professionals.

If you would like to make a referral you can either:

  • Download a Referral Form and either fax or post it back to us, or
  • Complete a referral over the phone with our Consulting Therapist

It is important that we gain an accurate picture of all your current wheelchair and seating needs and issues to assist with the referral process. We ask that all sections of the form are completed with as much detail as possible, to ensure that we have a good understanding of the issues.  Your information will be stored and handled confidentially.

 

Update a Referral


Mobility Solutions currently has a waiting list. Once we receive your referral we will post out a letter notifying that your referral has been processed.

You may find that while you are waiting to be seen, your needs may alter or become more urgent. It is important that you advise us of this as it will allow us to establish your level of urgency.

To update an existing referral you can:

  • Download a Referral Update Form and either fax or post it back to us, or
  • Update our Consulting Therapist.  They are available to take calls on Tuesday morning and Thursday afternoon.

Refreshments

Tea and coffee making facilities are available free of charge.  Please let us know if you need any assistance.  If you require special cups, please bring these with you.  Accessible bathrooms are also available.  Some appointments may be quite long.  Your therapist will let you know in advance and may suggest you bring along a snack

Public Transport

Rehab Plus is located approximately 1km from the Mt Albert train station.  We are also located near the Pt Chevalier shops which is a stop for many buses.  For more information we suggest you visit www.maxx.co.nz

Parking

Rehab Plus have a number of disabled parking spaces near the main entrance.  If you require assistance getting out of your vehicle or unloading it please let us know.
