Capital Support is a Needs Assessment and Service Coordination (NASC) agency that provides access to support services for people with disability related needs living in the Wellington, Porirua and Kapiti (up to PekaPeka) regions.
Support Assessment - is a process that helps people identify their disability support requirements. This involves a needs assessor meeting with a client, usually in their home, and completion of a Supported Needs Assessment Form. Support Assessment is necessary if you require access service coordination support and there is a need to access government funded services.
Service Coordination - is the process of selecting and organising the services required to meet the disability related needs that were identified during Support Assessment. This involves the development of a Support Plan which identifies what should happen to assist with maintaining or promoting client independence.
Capital Support has Maori and Pacific Needs Assessors/Service Coordinators who are available to work with Maori and Pacific clients.

Who is Eligible?
People with long term disability are eligible for needs assessment if they meet the following Ministry of Health definition:
"A person who has been identified as having a physical, intellectual, sensory (or a combination of these) disability which is likely to continue for a minimum of six months and results in a reduction of independent function to the extent that ongoing support is required."
Who is Not Eligible?
- People who are covered under the Accident Insurance Act 1998
- People who require assessment solely as a result of a mental health need
- People who require assessment as a result of a personal health need
- People who are not New Zealand residents
How to Access Needs Assessment Services
People with long term disability and/or their carers can access the service by completing a Capital Support Referral Form. The form can be completed by anyone provided it is done with the knowledge and consent of the person (or their carer) being referred. Please click here for a copy of the referral form.
Referral Process
The referral form is checked for eligibility and urgency and an appointment arranged with the client. The client can choose where the assessment takes place and can have a support person of their choice present.
