District nurses work with specialist nurses, allied health professionals and healthcare assistants, providing care to people in the community throughout the Wellington region. The service is available to people in their own home or at a clinic facility and care is provided based on an assessment of need. The staff have a range of professional, clinical and cultural skills and knowledge.
Services include:
- Acute nursing care (after hospital discharge)
- Oncology nursing, home-based chemotherapy services
- Assessment and treatment of complex and chronic wounds, Doppler assessment
- Home intravenous therapy
- Domiciliary oxygen and respiratory nursing services
- Stomal therapy services
- Continence services
- Palliative care services in partnership with Mary Potter Hospice (MPH )
- Breast cancer nurse services
- Enuresis programme
- Pulmonary rehabilitation programme
- Continence, stomal and oxygen consumables supply services
- Short term home support services, personal care and home help
Nurse led clinics
The nursing clinics are located at Kenepuru Commmunity Health Building, Kapiti Health Centre and the Wellington Hospital Outpatient Department.
