The Clinical Pathway Collaborative is a new programme which focuses on improving services and pathways for patients. This follows a series of timed meetings for clinicians from both hospital services and Primary Care, involving appropriate nursing, allied health and management personnel along the way. Patient representatives will also be consulted as developments require. The groups will develop implementation plans which will then deliver the changes and priorities needed. This will especially focus on the way that hospital and Primary Care services integrate and work together to improve the patient experience.
