Waitemata DHB ACC District Nursing

Description

The ACC District Nursing Service provides services to patients at home or at work, with injury related nursing needs that require the expertise and competency of a skilled, appropriately qualified nurse to promote rapid healing, rehabilitation and early return to work or participation in everyday activities.

 

Entry Criteria

The claimant is aged 16 years or over and:

  • has an accepted claim for cover for personal injury under the IPRC Act or is likely to have an accepted claim
  • requires community nursing services in their home or workplace in order to restore their health to the maximum extent practicable
  • lives or works within the WDHB catchment area.

 

Referral Expectations

With the patient's permission and in accordance with the Ministry of Health Specifications and the ACC Community Nursing Contract, referrals to the ACC District Nursing Service are accepted from ACC treatment providers such as:

  • general practitioners
  • DHB staff
  • medical specialists
  • ACC assessors
  • self, family and caregivers in partnership with other healthcare providers.
