Te Kaahui Ora - Maaori Health Services
Our service includes the Whaanau Support & Hauora Whaanau Teams.
Whaanau Support Team
Are a team of Whaanau Support Workers who provide inpatient support to Maaori patients (and their whaanau) who have been admitted into Middlemore Hospital.
Their priorities are to:
- Facilitate access for whaanau into our Emergency Accommodation and provide ongoing monitoring and support during their stay.
- Work as an integral part of the multi-disciplinary team to provide culturally appropriate support to Maaori patients and their whaanau.
- Ensure that Maaori patients and their whaanau are fully informed of and understand the process of care that will occur during their inpatient stay at Middlemore Hospital.
- Ensure that Maaori patients and their whaanau are aware of and have access to resources within the various wards and the wider hospital services.
- Uphold cultural safety and best practice by advising and guiding staff on culturally appropriate interventions with patients and their whaanau.
- Work with the multi-disciplinary team to ensure that Maaori patients discharged from hospital have an appropriate discharge plan in place.
Hauora Whaanau Team
Are a team of Social Workers and Registered Nurses.
Areas Covered by the Team
- Kidz First Emergency Care
- Kidz First Medical & Surgical Care
- Kidz First Neonatal Unit
- Kidz First Homecare Nurses.
The Hauora Whaanau Team Will
- Provide a culturally safe service
- Provide education and health promotion strategies
- Facilitate appropriate and effective co-ordination between services
- Monitor and review whanau engagement with referred services
- Advocate on behalf of whaanau.
