The CSW Service offers home-based practical support to clients with enduring mental illness.
To be eligible for a Community Support Worker, clients must be under the care of a Community Mental Health Centre and be living independently, but not necessarily alone, in the community where lifestyle has been disrupted for the last 6 months or more as a result of their mental illness.
The CSW service provides both 1:1 support as well as a variety of group programmes which focus on assisting the individual to identify and develop the necessary skills required to improve their quality of life in the community.
Support provided by Community Support Workers includes:
- accessing work, study and leisure opportunities
- maintaining suitable housing/accommodation
- accessing appropriate benefits and support
- identifying personal strengths and setting achievable goals.
