The Inpatient Hospital Pharmacy provides pharmaceutical care to patients in hospital.
Clinical pharmacists visit the hospital wards on a regular basis to provide medicines management and pharmaceutical care to patients.
This involves consultation with patients and their caregivers if necessary. All patients are encouraged to bring their medicines into hospital with them to ensure we are able to tell exactly what medicines are being taken.
The pharmacy team works very closely with the medical and nursing staff to identify and resolve any problems with medication.
Individual patients may receive additional review and medication counselling on request of the medical team or as deemed appropriate by the pharmacist.
Inpatient pharmacies are at both North Shore and Waitakere Hospitals.
North Shore Hospital Pharmacy - Outpatient Pharmacy provides medicines management to patients on discharge from hospital who are provided with a discharge prescription.
