Waitemata DHB Maori Health Services - Mo Wai Te Ora

Hours

Services operate from Monday to Friday, 8am - 5pm.

Description

'Mo Wai Te Ora?' Ka kotahi te haa o te kaumatua me te mokopuna. He tohunga hauora nei whakatutuki ki te iwi Maori o Te Wai Awhina, ko te kawa te hauora maori me ona whakaritenga i te wairua, tinana, hinengaro me whanaungatanga, te pou pou mo te hanga te tuku oranga, kua enei te wairua o Te Tiriti o Waitangi.

 

This team supports inpatients in North Shore and Waitakere Hospitals and includes: Kaumatua, Nurses, Social Workers, Health Promoting School Co-ordinators, Parenting Programme Co-ordinators, CAYAD and Healthy Lifestyle Planners.

 

Services we offer for inpatients and their families include:

  • cultural/clinical assessment, designed to help Maori patients and their whanau during their stay in hospital
  • case management, social work support and advocacy
  • tikanga and bereavement Support for families of inpatients
  • help and support for people if your language is Maori
  • organising overnight accommodation for whanau using North Shore Hospital services.

Referral Expectations

If you are a patient at either of the hospital sites and if you identify yourself as Maori, you are welcome to call on our services. Please ask your nurse for a referral to the Mo Wai Te Ora Maori Health Service, by faxing (09) 441 8971

We accept referrals to assist patients, but we are not a 24-hour service. 

During office hours, please contact our Charge Nurse directly on (09) 486 8324 (internal ext 2324) then send written referrals by fax to (09) 441 8971.  If you cannot access our Charge Nurse, then please call our Administration on (09) 489 0553 (or internal access x3553)

After hours,  urgent telephone referrals should be made to the Hospital Duty Manager, by calling (09) 4861 491.  Written referrals or letters to refer a patient should be faxed to (09) 441 8971.
