Community Dietitians

Community Dietitians are key members of the Home Health Care (HHC) Team, a community-based healthcare service covering the large Counties Manukau Region. An important focus of the HHC Team is to provide education to empower people to lead healthier lives and support them to continue to live in their own homes.

Locations
There are four community-based sites located at Howick (Botany), Orakau Road Mangere, Papakura, Pukekohe and a sub-base at Waiuku. The service is available to people in their own home or at a clinic facility, and care is provided based on an assessment of need.

Who Do We See?
Community Dietitians work with adults (over18 years of age) who have health conditions requiring highly specialised nutritional care. These include people on home enteral nutrition (tube feeding); the frail elderly, and people with chronic neurological conditions such as Motor Neurone Disease and Parkinson’s disease. Community Dietitians also provide nutrition support to people with cancer who are undergoing active treatment, particular those with head and neck cancers. All patients are expected to have a nominated GP.

Outpatient Clinics
Clients who are mobile and who have access to transport will be seen at an outpatient clinic (Orakau Road, Manukau SuperClinic™ or Botany SuperClinic™).

Service Provided
The Community Dietitian works closely with the client and their family / whanau in their own home (in the case of housebound individuals) or at outpatient clinic settings. The nutrition care process includes nutrition assessment, goal setting, planning a nutrition care plan and ongoing nutrition monitoring. 

Referrals
Referrals are accepted from General Practitioners (GPs) and other health professionals, provided that the individual has consented to the referral.

Community Dietitians Do Not See:

  • Rest Home or Private Hospital residents
  • Clients whose needs are met by other services e.g. Renal, Diabetes, Mental Health or Gastroenterology 
  • Terminally ill people (who are under the care of Hospice)
  • People requiring advice for weight management or general dietary advice. We recommend that these clients consult with a dietitian in private practice.
