Introduction
Long-term conditions have been described as the “healthcare challenge of this century”. The World Health Organisation estimates that globally 60 percent of all deaths are due to long term conditions. Over the next ten years the number of deaths attributable to these conditions is projected to rise by 17 percent (WHO 2005). Long-term conditions are the leading cause of death in New Zealand, and account for more then 80 per cent of the deaths. It is expected that a focus on long term conditions would contribute significantly to reducing inequalities in health.
A framework and plan are currently being developed with input from across the sector.
