The role of a GP Liaison is to work with both primary care (GP services) and the hospital to improve services for patients when they are moving between these two providers of healthcare.
Typically it involves work around:
- improving transfer of patient information between the two groups
- ensuring hospital services are designed for ease of use by primary care
- ensuring there is good information to primary care about hospital services
- facilitating solutions to problems experienced by patients that are referred to the hospital by their GP.
Northland DHB has two GP Liaisons working part time.
