Description

Affinity Services is a standalone Charitable Company, owned by the Affinity Services Charitable Trust. We are a community based organisation and have provided mental health services in the Auckland region since 1975.

Affinity provides community based, recovery focused, mental health support services and strives to assist people attain and live rich and fulfilled lives. We are authorised trainers in the Strengths Model and we work with people using the following recovery tools: Prof. Charles Rapp's Strengths Model; Prof. Mason Drurie's Te Whare Tapa Wha (Four Cornerstones) Model; and Mary Ellen Copeland's Wellness Recovery Action Planning (W.R.A.P.).

Community Living Service

Eligibility:

Referral Process:

 Community Support Service - Mainstream

Eligibility

  • Anyone over the age of 18 who has a mental health diagnosis, such as:
    • Schizophrenia
    • bipolar disorder
    • depression or
    • anxiety disorders.
  • people can access this service provided they have a current clinical key worker.

Referral Process:

  • Referral to this service can be made through a Community Mental Health Service (CMHS).

Community Support Service - Asian

Crisis Respite

Eligibility

Referral Process:

  • All referrals are through the Counties Manukau Clinical Teams in the district.  To access a clinical team you will need your GP to make the initial assessment and referral.

Housing & Recovery Service - Residential Rehabilitation

Eligibility:

Referral Process:

  • Referrals are made only via clinical teams and service coordinators.

Maternal Mental Health Crisis Respite

Eligibility:

  • For mothers experiencing mental health crisis, postnatally.
  • We are also able to work with expectant mums with pre-existing mental health issues.
  • Lives in the Counties Manukau DHB region

Referral Process:

All referrals are through the Counties Manukau Maternal Mental Health Service.  To access a clinical team you will need your GP or midwife to make the initial referral.

Muslim Mental Health Service (under development)

Referral Process:

This service provides mental health promotion and awareness to the central Auckland Muslim community.  For more information on this service please contact Khalid Shah mobile number: 027 484 3122.

Reframe Youth Respite Service

Eligibility:

  • For Youth 16-24 years experiencing mental health crisis.
  • Lives in the Auckland DHB region

Referral Process:

All referrals are through the Auckland DHB Community Mental Health Centres or The Kari Centre Community Mental Health teams.  To access a clinical team you will need your GP to make the initial referral.

Specific Cultural Support

Eligibility:

  • Maori or South Asian who have been diagnosed with mental illness
  • aged 18 years to 65 years
  • live within the Counties Manukau DHB region

Referral Process:

  • Referrals can be made by: self referral, family or friends, psychiatrist, key worker or GP, other professionals or service providers.
