Please ensure your referrer has faxed through all necessary documentation prior to your appointment such as:
- Any letters or reports from your doctor or hospital.
- Any X-Rays, CT or MRI reports.
- A list of medicines you are taking including herbal and natural remedies.
- Your ACC number and date of injury.

Free patient parking is provided at each consulting clinic.
For Ascot Office Park, parking is available in the basement to the left of the building. Please use parking spaces marked as MEDICAL CENTRE only which are situated on the right wall of the car park in front of the 1st lift.
Please Note - parking can be limited and restrictions are strictly enforced. Additional parking is available across from the building which services Ascot Hospital. Normal parking fees apply.
