Our holistic care and support extends to family/whānau and carers.  We provide practical advice and physical, emotional and spiritual care and support.

Our focus is on quality of life, comfort at the end of life, and helping those who have lost loved ones.

Referral Expectations

Referral Guidelines

People can access specialist palliative care services if:

  • They are assessed & accepted as having a life-limiting condition with a life expectancy of less than 12 months.

Referrals will be accepted from:

  • General Practitioners, other healthcare providers, hospital clinical teams, residential care facilities etc.
  • A patient or their family / whānau / carers

 Notes:

The patient must consent to referral.

Procedures / Treatments

Specialist Community Palliative Care

Our specialist Palliative Care team works holistically with the patient and the family of the patient. Their work is mainly in the community, caring for patients living at home or in residential care. Regular nursing clinics are also held at Hospice House in Beach Road. Scheduled visits are made during normal business hours. After hours visits or phone support is available. Inpatient hospice care is accessible for complex and/or high specialist palliative care needs. Hospice West Auckland exists so that people with life-limiting illness in West Auckland, those at the end of life and those who grieve for them, experience death as a dignified and compassionately supported process. You are welcome to call us for advice.

Family Servies

Family Servies offers emotional, psychological, practical and spiritual support services for patients and their family/whanau. The team comprises: Social Work, Counselling, Chaplaincy, Complementary Therapies, Art Therapy and Day Programmes. Appointments are available for individuals, groups and families.

Visiting Hours

Hospice House is open Monday to Friday between 8.30am and 5pm.  Hospice West Auckland is a community Hospice.

Parking

Limited on site parking, but plenty of free parking on the street.  Drop off bay at the front door.

Hospice West Auckland provides services free to the people of greater West Auckland.

We are partially funded by the Waitemata District Health Board, with the balance of operating revenue required coming from donations, fundraising events, our annual appeal, trusts and grants, sponsorship and The Hospice Shops.
