Cardiac Rehabilitation

Cardiac rehabilitation is provided at Auckland City Hospital to support, educate and assist the patient, spouse, partner, whanau and family following an acute cardiac event.
By their own efforts, patients may resume normal functioning in society and through improved health behaviours can also slow or reverse the progression of cardiac disease.
 
A team of health professionals is involved in cardiac rehabilitation including:
4 Nurse Specialists (CRNS), 2  Psychologists, 2 Physiotherapists, a Smoking Cessation Specialist, a Social Worker, a Pharmacist and a Cardiac Consultant.
 
Phase I consists of the information, education and support a patient receives at the bedside in hospital to aid understanding of heart disease, also providing information on how risk factors can be modified. This assists towards self-care, physical and psychological recovery.
 
Phase II is a supervised period of six months, consisting of periodic phone follow-up after discharge from hospital, attendance at an Education Programme and an invitation to take part in a specialised Exercise Programme.
Phase II also involves contact with the relevant health professionals to assist each individual with their own individual risk factors for heart disease.
 
Outpatient Programmes

For Medical Patients  (MI, angina, heart failure)

Phase II Cardiac Education at 100 St Andrews Road, Epsom. Seven sessions, every Tuesday 1.30pm - 2.30pm.

Course content: exercise for heart health and future, risk factors for heart disease, emotional impact and stress management following a cardiac event. Also understanding medications, healthy eating, and meet and question the cardiologist.

For Surgical Patients  (post CABG and valve surgery)

Phase II Cardiac Education at Ward 42, Auckland City Hospital, Park Road, Grafton. Seven sessions, every Monday (except public holidays) 12.30pm - 1.45pm.   

Course content: exercise for heart health and future, risk factors for heart disease, emotional impact and stress management following a cardiac event. Also understanding medications, healthy eating and meet and question the cardiologist.

Evening Programmes 

Usually held on 2 consecutive Monday evenings 3-4 times a year.  This programme is specifically designed for patients and family who are unable to attend the day programmes due to work and family commitments.

Exercise Programme

Wednesday and Friday, 1.00pm - 3.30pm for eight weeks.
Held in the Physiotherapy Department at the Greenlane Clinical Centre.

Content: individual assessment and exercise programme progression. Pre-assessment necessary.
