A+ Links Home Health

Hours

District Nursing service is available from 08.00am - 9.30pm, 7 days a week including Public Holidays.
Allied Health Services are available Monday to Friday 08.00am - 4.30pm.
Home Aid Service is usually provided Monday-Friday 8.00am to 4.30pm. For those who meet the criteria for out of hours personal care this service may also be offered at weekends.
Meals on Wheels service is available Monday to Friday only not on weekends or Public Holidays.

Description

A+ Links is an integrated hospital/community service made up of Home Health and Older People's Health (OPH) for those over 65 years old.

A+ Links Home Health service provides community based healthcare. Home Health offers home based care for adults 16 years and over covering all of Central Auckland.
 
Home Health provides:
  • Nursing Services
    District Nursing, Oncology, Leg Ulcer Service, Gerontology, Continence, Ostomy
  • Allied Health Services / Day Assessment Clinics
    Occupational Therapy, Speech Language Therapy, Social Work, Physiotherapy, Physiotherapy – Falls Prevention Programme, Dietetics, Community Rehabilitation Team (CRT) – Stroke & Specialist Rehabilitation
  • Home Services
    Provide short term home help to people who have had an acute illness, surgery or have oncology related health needs. This help depends on personal circumstances and includes housework, personal care and assessment for Meals on Wheels.



Care Aims
a) To improve the quality of life of individuals and families/whanau 
b) To improve the health of the population of central Auckland and assist people to remain well at home.

To do this:
- We are committed to strengthening our relationship with other primary, secondary and tertiary care providers.
- We provide personal health care and disability support services to people whose health needs can be met at home or community venues.
- All of our services have a coordinated, multi-disciplinary approach and our staff work towards culturally appropriate service delivery. Interpreters are available when required. 

Referral Expectations

All Home Health staff coming to visit you at your home will introduce themselves and be wearing photo identification with their name and role when providing service. They will explain treatments, procedures and their likely effects. Please ask if you are not sure about anything that is happening to you.
Please, secure your dogs when expecting a visit from Home Health staff.
 
Health information is collected in order to provide appropriate care and treatment. Information may also be used for administration or teaching and research purposes. All members of the Home Health team who are involved in your care will share your health information.
 
Should interpreters be required for people who do not speak English as a first language, they can be arranged by the staff member who visits you.
 
Health Advocates are available to support you. The Health Advocacy Service is an independent service, which is associated with the Health and Disability Commissioners office. Advocates can be contacted by phoning (09) 638 9638.

Charges

New Zealand citizens or people who have obtained permanent residence are entitled to free health care. Persons not ordinarily resident in New Zealand will be required to pay the full cost of their treatment. If you are a non-New Zealand resident, or not born in New Zealand, please bring your passport to your appointment.
 
There is a charge for some equipment, supplies and the installation of handrails. This will be discussed with you at the time.
 
Please NOTE these CHARGES:
Home Health Physiotherapy does not have a contract to provide rehabilitation for clients who are registered for their conditions with ACC.
 
Home Health Occupational Therapy: Funding for equipment and housing modifications can be recommended by the Occupational Therapists when you meet the disability criteria. The Occupational Therapist will discuss and provide all relevant advice and information with you and your caregivers.
 
Home Health Home Service - Meals on Wheels: There is a charge of $4.00 (including GST) per meal paid monthly on invoice or by automatic payment.
