Description

Assessment, Care Planning and Coordination for Community Based Health Services

The Care Coordination Centre will help you if you need community support to maintain your health and independence. The Centre will coordinate access to community based health services for people living in Kapiti, Porirua and Wellington. These include services provided by Capital & Coast DHB staff as well as providers who are contracted by the DHB.

The Centre is operated on behalf of the Capital & Coast DHB by Nurse Maude, a community health organisation.

The Care Coordination Centre is the one point of entry for referrals requiring community based supports.

 

Referral Expectations

You can be referred to the Centre by your GP, the hospital or a health professional or you or your family/whanau can contact the Centre directly.

On receipt of a referral we may telephone you to make sure we understand what your needs are. For those clients who have less complex needs your referral will be forwarded to one of the CCDHB contracted providers who will organise an assessment in your home.

If your needs are more complex a full assessment of your needs will be undertaken by a health professional (Care Manager) who will visit you and develop a care plan that will enable you to live as independently as possible and achieve your health goals.

Services

  • Home Based Care & Support Packages

    Once an assessment has been completed and your support needs identified a CCDHB contracted home based support provider will visit and organise services to commence. More

  • Aged Residential Care

    Staff at the Care Coordination Centre will be able to assist you with any enquiries you may have regarding residential care. More

  • Respite Care

    Once an assessment of your needs has been completed (or that of your spouse, relative or family member) a respite allocation may be made.… More

Charges

The Centre's services are free to NZ residents. Most health and disability services you are likely to receive will be free but, if there is a charge, the Centre will discuss this with you.
