Counties Manukau Health Adult Rehabilitation Service (Neurological)

Description

The Adult Rehabilitation Service provides assessment, treatment and rehabilitation for patients living in the Counties Manukau Health region.

Care is provided by an interdisciplinary team, that develops a patient-focused, goal-based treatment programme, based on the individual patient's needs.

The service includes inpatient care on Ward 23, Middlemore Hospital, an outpatient medical and physiotherapy service based at the AT&R Unit of Middlemore Hospital and a community stroke rehabilitation service.

Referral Expectations

The criteria for admission for an inpatient period of rehabilitation are:

  • Reside within CMDHB catchment area
  • Aged 16 upwards
  • Medically able with medical potential for full participation in rehabilitation programme
  • Willing to participate in an individualised rehabilitation programme
  • Potential for improvement and good rehabilitation outcomes

Procedures / Treatments

  • Acute Consultation Service

    Patients referred for admission to the inpatient Adult Rehabilitation service after an acute episode are assessed by a rehabilitation consultant or geriatrician to determine the appropriateness of their admission to the service.  These patients may be referred from other services in Counties Manukau District Health Board, including the community services… More

Charges

No charge for the service for New Zealand Residents.  Non residents are charged a per bed day rate.
