Wellington Hospital Pharmacy service supplies medications to all inpatients at Wellington, Kenepuru and Porirua Hospitals.

Pharmacists provide a clinical pharmacy service to all the inpatient wards. The pharmacists review medicine charts, monitor drug therapy and facilitate the supply of medicines. Clinical pharmacists also advise on safe and effective use of medicines.

The pharmacy service also provides medicines information to other health professionals and patients and, where appropriate, pharmacists advise patients on how to best use their medicines. Other services include aseptic compounding of medicines and clinical trial services.

Wellington Hospital Pharmacy does not dispense outpatient prescriptions except for a restricted range of medicines that are not usually available from the community pharmacy.

Clinical pharmacists visit the inpatient wards on a regular basis and manage medicines and provide advice on pharmaceutical care.

Patients are encouraged to bring their medication or medication lists, such as yellow cards, into hospital with them so that we can ensure the correct information on medicines is available at the time of admission. We also recommend that complete current medication details are provided on referral letters, including dose and frequency of medications and any allergies or previous problems with medicines.

Pharmacists may call the patient's GP or pharmacy to determine the medicine history or to clarify any inconsistencies. Any changes to medications during the patient's stay are normally discussed with the patient and details of changes are often provided in the discharge summary.
