Formerly Counties Manukau Health Pharmacy

Inpatient Pharmacy: Middlemore Hospital Pharmacy supplies medications to all inpatients of Middlemore Hospital and other Counties Manukau Health facilities (Manukau Surgery Centre, Franklin Memorial Hospital, Pukehohe Aged Care). 

Community/Outpatient Pharmacy: Haumanu Pharmacy is located in Middlemore Hospital in Pataka Place. 

Clinical Pharmacy - Pharmacists provide a clinical pharmacy service to all inpatient wards at Middlemore Hospital.  Clinical pharmacists work closely with patients, doctors, nurses and other health professionals and advise on safe and quality use of medicines.  Pharmacists establish medication histories, review medication charts, monitor drug therapy and facilitate supply and access to medicines.

Medicines Information Service - The Medicines Information Service consists of a team of experienced, senior pharmacists who are available to answer medication-related enquiries for health professionals in the CMDHB region. The team are available on Monday to Friday from 8am - 4:30pm.

Referral Expectations

Clinical Pharmacists visit inpatient wards and some Counties Manukau Health outpatient clinics to provide medicines management services and advise on pharmaceutical care.  Patient services such as medication review and medication counselling are part of this process.  

Patients should bring any current medication (tablets, creams and/or inhalers) into hospital with them on admission as inpatients.  Medication lists, such as yellow cards, are also encouraged.  This ensures that the optimal information about medication being taken is available to the health team at the time of admission.  Pharmacists may call a patient's GP or regular community pharmacist to confirm or clarify differences in medication history.
