Description
Brightly is a geriatrician-led healthy ageing service supporting independent adults aged 65+ to maintain health, function and confidence as they age.
Our comprehensive Baseline+™ assessment provides a structured, whole-of-person evaluation across 16 domains of health and ageing – including cognition, mobility and balance, falls and bone health, polypharmacy and medication review, cardiovascular risk, mood, sleep, nutrition, continence, hearing, vision, legal readiness and financial resilience.
Following assessment, members receive a structured Brightly Summary Report outlining strengths, risks and priority areas. Brightly provides ongoing support for identified health priorities, along with preventative guidance through our membership programmes.
We welcome GP referrals, as well as self-referrals and referrals from family members.
Staff
Our multidisciplinary team includes geriatricians, GPs, nurses, physiotherapists, occupational therapists, pharmacists, dietitians, psychologists, and health coaches.

Brightly HealthPlan™:
- Lite: $90 per month (incl GST)
- Core: $200 per month (incl GST)
- Plus: $400 per month (incl GST)
