How to access this service
Patients are referred through their hospital, GP or health professionals working in the community or from Northland DHB (aged over 65 or for support following a stay in hospital), NorthAble (aged under 65 with disability) or ACC (following an accident).
Assessment is completed by trained needs assessors. (If you’re not entitled to funded care through the Ministry of Health, a District Health Board or ACC, we can talk about other options, including private care.)
We have a team of co-ordinators in each office who are qualified nurses, EN or RNs and also Co-ordinator Administrative Support.
