~ Northland’s largest Home Based Support Service provider – dynamic, friendly team of Nurse

Co-ordinators and Support Staff ~

Home Help: this may include housework, meal preparation, shopping, laundry.

Personal Care: this may include showering, bathing, feeding, transporting to appointments, grooming, dressing, assistance with mobility/transferring.

Some Child Care available through ACC if you have had an accident.

Free assistance can be allocated by needs assessors for someone to come into your home and look after you or your dependent loved-one so you can have a break.

Staff

We have a team of Nurse Co-ordinators who are qualified nurses, EN or RNs and also a Co-ordinator for Administrative Support.

How do I access this service?

Referral

Patients are referred through their hospital, GP or health professionals working in the community or from Northland DHB (aged over 65 or for support following a stay in hospital), NorthAble (aged under 65 with disability) or ACC (following an accident).

Assessment is completed by trained needs assessors. (If you’re not entitled to funded care through the Ministry of Health, a District Health Board or ACC, we can talk about other options, including private care.)

Hours

7:00 AM to 5:00 PM.

Mon – Fri 7:00 AM – 5:00 PM

Mid/Far North Office (Kerikeri) open 8am to 4pm Monday to Friday, excluding Public Holidays.

Monitored answering service operates outside regular business hours - 5pm to 9pm weekdays, 7am to 9pm weekends and Public Holidays.
