Kahungunu Executive ki Te Wairoa Charitable Trust is a Māori health provider delivering health, mental health, mobile nursing, social and housing services to the Wairoa community and surrounding rural areas.

Description

This service provides non clinical care and support for people in the community with Oranga Hinengaro needs.

Our services are kaupapa Māori based and we use tīkanga, including the Whare Tapa Whā; Māori model of Practice, to assist with the recovery and support of Tangata Whaiora / consumers in the following areas:

The individual short and long term support services include: cervical smears, breast screening referrals, lung function assessments hearing assessments, Green Prescription referrals health assessment and monitoring identifying clients and whānau at risk of illness or injury health and social service referrals support and management of acute or chronic medical conditions discharge from hospital follow up care in association with hospital and GP services.

Health Promotion

This service aims to create awareness around healthy lifestyle choices and wellbeing of individuals and whānau to manage and improve their health. The core focus is on Nutrition, Physical Activity and Smoke Cessation. Our team also includes Injury Prevention/Car Seat and Mirimiri services.

Strengthening Families

Strengthening Families is for children, young people and their families/whānau who need support from more than one agency. It brings together the family/whanau and all the agencies that have a part to play in helping the family/whanau achieve its desired outcomes. It is a structured, early intervention, family/ whānau-centred process.

Whānau At Risk

We support children, young people and their families/ whānau who are affected by family violence. As a result, everyone feels stronger, more connected and supported by their communities.

Social Worker in Schools: To provide social work support for vulnerable children in years one to eight (5-12) and their family/whānau.
