LifeLinks provides personalised, whole-of-life planning and coordination services to support people with disabilities. We work alongside you and your family/whānau to understand what matters most to you and to develop an Outcome Plan that reflects your goals and aspirations.
Using a person-centred approach, we look at key life areas to understand what’s working well and what could be improved. Together we identify a mix of supports including natural, mainstream, and funded options, that may help you achieve your identified outcomes.
LifeLinks is contracted by MSD DSS to provide Disability Support Services planning and coordination for people with physical, intellectual, sensory, or neurological disabilities aged 0–65 years in Canterbury, South Canterbury and the West Coast. There is no cost for accessing outcome planning and coordination services.
We recognise that family/whānau and carers are central to wellbeing, and early access to support can help maintain resilience and quality of life. Ask your outcome coordinator about other services and agencies that can provide families/whānau/carers with information, advice, peer support and social networks.
