Description

THINK Hauora delivers a range of local and national health and hauora services and programmes to whānau living within the MidCentral rohe.

Our programmes and services are tailored to the needs of our local communities, and include support for people with long-term conditions (such as Better Breathing and My Health Myself), helping people build confidence and meaningful employment pathways through Here Toitū, and mental health and wellbeing support through Te Ara Rau Access & Choice — all aimed at supporting whānau to improve their wellbeing. Services are delivered through our offices and in our community clinics.

Immunisation

Immunisation is the safest and most effective way to provide protection for you and your tamariki’s health. For more information view the NZ immunisation schedule. Immunisations - Our clinical team offer a number of immunisation programmes including Childhood Immunisations, MMR and the Flu vaccine. You can access these immunisations via one of our regular drop in clinics. Read more here

My Health Myself - A group-based, six-week course with other like-minded people wanting to learn more about managing their long term condition. Community health screening

Retinal Screening Service – People with diabetes are at risk of developing eye damage called diabetic retinopathy. When a person has been diagnosed with Type 2 diabetes, the General Practice Team will refer them to an Optometrist to monitor any changes. This examination should be completed every 2 to 3 years. Health screening tests check for health conditions or early warning signs of disease.

Te Ara Rau - Access & Choice

It doesn’t matter what’s causing you worry - our Access & Choice team is here to help. It’s completely normal to feel overwhelmed at times, or to notice your thoughts, feelings, or actions starting to affect your health and wellbeing. You don’t have to navigate that on your own. Our Access & Choice team can work with you to make sense of what’s going on, explore what you need, and support you to get back on track. Our Access & Choice service is free to access.

Service types: Counselling, Education and training, Social work.
