Diabetes Nurse Educator Services (DNE):
The PHO engages a DNE to provide education for Practice Nurses to upskill their clinical knowledge to provide best practice care of people with diabetes.
Diabetes Management:
The PHO supports its general practices to care for people where their diabetes who require additional management through their GP or practice nurse.
Diabetes Annual Review & Diabetes Management
The PHO funds over 6000 'diabetes annual reviews’ for those diagnosed with diabetes. These reviews are undertaken in general practice.
