Eastern Bay Primary Health Alliance (EBPHA) is a Primary Health Organisation (PHO) that is funded by Te Whatu Ora and others to support the provision of essential primary health care services through general practices and health providers to those people who are enrolled with EBPHA.
EBPHA provides services either directly or through its provider members. These services are in place to improve and maintain the health of the entire enrolled PHO population, as well as providing services in the community to restore people’s health when they are unwell.

Self-Management for Heart Health and Diabetes: Looking after yourself is one of the most important things you can do in your lifetime. Self-management sessions mean you can take control and do something to ensure you remain well. Eastern Bay Primary Health Alliance runs Heart Health Self-Management Sessions and Diabetes Self-Management Sessions. Self-Management sessions run for two hours per week, over a four week period. They are FREE for both you and a support person to attend and are designed to provide information and education to improve your health and reduce your risk factors. Contact Chrissy Simmons. Our brief intervention service is private and confidential and is an opportunity for whanau and our counsellors to work together, explore and develop personal strengths, so they can find their own solutions to issues they may be facing. We offer sessions for our adult whanau (20+ years) and our youth (10-19 years). Our service supports mild-to-moderate issues including anxiety, depression, grief and loss, phobias, and more. Referrals for adults (20+ years) must be from the GP. For youth (10-19years) you can self-refer, whanau, schools, other organisations, your GP can refer. Contact: Freephone 0508 742 7642

The ICM service supports General Practice in the care of patients with chronic care conditions and/or complex needs by providing case management and navigation services that support and compliment the clinical objectives. The ICM team are able to assist with: Advocacy and support; Referrals to social and community health services; Home visiting; Telephone monitoring; and Self-management support. Referrals to the service can be made through General Practice only via Med Tech e-Referral.

Most Childhood immunisations are undertaken by your GP or nurse at your General Practice. For those who are unable to attend General Practice, an outreach service is available. Outreach Immunisation: Outreach Immunisation Services (OIS) are for children and families/Whanau that are not accessing immunisation services provided by General Practice. These services are provided in an attempt to immunise those children who have been identified as having missed some or all of their scheduled childhood immunisations. OIS are one of an Integrated set of strategies aimed at improving immunisation coverage and ensuring that people have access to services that empower them to make informed decisions with regard to immunisation. Who is eligible for OIS? All children birth-6 (inclusive) years who have not accessed scheduled immunisation services and where primary health care services are unable to either locate them or have not been successful in bringing the family/Whanau into the primary health care service for their immunisation. How are these services provided? OIS offer flexible arrangements for immunisation services i.e. by providing services both in the home or community settings. Who is eligible for OIS?
All children birth-6 (inclusive) years who have not accessed scheduled immunisation services and where primary health care services are unable to either locate them or have not been successful in bringing the family/Whānau into the primary health care service for their immunisation.
How are these services provided?
OIS offer flexible arrangements for immunisation services i.e. by providing services both in the home or community settings. At the EBPHA, the Immunisation Advocate supports and accompanies the Outreach Vaccinator into the community, following up and providing information for families who have children that have missed vaccination.

Sport Bay of Plenty's trained staff support individuals to achieve goals in relation to increased activity and weight loss. Our Counselling and Hapainga Stop Smoking Services are also available to engage with groups in schools.
