Self-Management for Heart Health and Diabetes:
Looking after yourself is one of the most important things you can do in your lifetime. Self-management sessions mean you can take control and do something to ensure you remain well.
Eastern Bay Primary Health Alliance runs Heart Health Self-Management Sessions and Diabetes Self-Management Sessions.
Self-Management sessions run for two hours per week, over a four week period.
They are FREE for both you and a support person to attend and are designed to provide information and education to improve your health and reduce your risk factors.
