Integrated Case Management (ICM) Service
Service types: Advocacy, Family / whānau support, Home support, Information & support.
The ICM service supports General Practice in the care of patients with chronic care conditions and/or complex needs by providing case management and navigation services that support and compliment the clinical objectives. The ICM team are able to assist with:
- Advocacy and support;
- Referrals to social and community health services;
- Home visiting;
- Telephone monitoring; and
- Self-management support.
Referrals to the service can be made through General Practice only via Med Tech e-Referral.
