Accessing the service is as easy as picking up the phone.  Hospice accepts referrals from patients, GPs, immediate family, hospitals, specialists, social workers etc (as long as the patient consents to the referral).

A hospice nurse  will contact the patient and set up an appointment for an assessment.  The nurse will assess the patient and either accept the referral or explain why they do not meet our criteria. 

The nurse will explain the role and involvement of the organisation in their care.

Patients are welcome to have family and/or carers with them at this appointment.
