This service provides support for patients living with two or more chronic conditions for example, heart disease, cancer, gout, deep vein thrombosis, and those requiring medication education.
Our nurse provides education and follow up visits before and after surgeries or treatments, and is able to support clients to attend specialist, GP and WINZ appointments.
Our nurse will provide education on your illnesses in a way that you understand in a place you are comfortable including on local Marae, in your home, in our consult rooms, or anywhere else that suits you.
Our nurse is able to refer you to other specilaised services such as podiatry, dietician, green prescription, district nursing, Support Nett, Occupational therapy, respite care, hospital services, mental health and addiction services and our Whānau Ora service.
The Pouwhenua service can also support whānau to register with a GP service in your area and to use your community as a resource.
The nursing service area covers Te Puke, Rangiuru, Manoeka, Pukehina, Pongakawa, Maketu and Otamarakau.
