Description
The SME (self-management education) programme is available to patients who are enrolled in a GP practice in Counties Manukau and have long term conditions including genetic, diabetes and pain specific conditions particularly Māori and Pacific patients.
The DSME (diabetes self-management education) programme is available in Waitemata for people with diabetes (Hba1c over 64 mmol/ml) particularly Māori and Pacific patients who are at risk of complications.
Both the SME and DSME programmes support people to cope better with the challenges of their health issues. They learn tools and methods to live well and improve their condition.

Referral
Self referrals are accepted otherwise you can be referred by a health professional e.g. practice nurse, doctor or physiotherapist.

This is a free service and is open to anyone over 18 years old living in Counties Manukau.
