Dietetics: QE Health has a contract with RAPHS for clients with a long term condition who are newly diagnosed diabetics or struggling to control their diabetes.
Referral is through the RAPHS Long Term Conditions Group.
Dietetics can also be accessed privately - no referral required, phone for an appointment.
QE Health also offer Green Prescriptions - a low cost gym membership for those clients referred by their doctor.
