Kaupapa Māori

Kaupapa Maori Community Based Nurse-Led Service:

Focus on clients, both adults and tamariki, who have long term conditions and high risk factors.

The service covers the Whakatane area and wider community (Kawerau, Matata, Edgecumbe, Ohope).

Our services provide nurse led clinical care that offers support/education to individuals & whanau around long term conditions such as:

  • Diabetes
  • CVD risk/ cardiac conditions
  • Respiratory conditions

This support/education is specific to health conditions, lifestyle, medications and health screenings are also provided.

Services can be home based or in a clinical setting.


This free service is available for babies, children and adults whom reside within this region.

Must have two or more chronic long term conditions e.g. diabetes and cardiac conditions.

Have an acute exacerbation and live rurally, therefore have additional barriers to accessing health and have an increased risk of hospitalisations.

    Tamariki Ora - Well Child

    The home based service delivers tamariki ora Well Child checks to children aged between 0-5 years old who live in the Ngati Awa rohe.

    • Seven entitled Well Child checks
    • Clinical assessments
    • inform and support parents to gain knowledge and skills to understand and manage stages of their child's development
    • promote positive parenting skills
    • facilitate access to other community health services
    • advocacy
    • whanau needs are supported
    • health promotion and education


    Te Oranga Kaumatua o Ngati Awa

    Service dedicated to Kaumatua to encourage social interaction and self development.  Delivered in a kaupapa maori setting, the day programme is two days per week includes outings in the community, health assessment, education and promotion through community presenters and social activities. A one day home visit service is provided to those unable to get to the day programme.
