Whanau care and independence services

Tuwharetoa Health provides a comprehensive range of services to support you and your whānau. These services can work together in a complementary way to provide a wraparound package of support that can help elderly whānau to stay independent in their own homes for as long as possible.   • Home Based Support Services – working with clients who are assessed as requiring support in their own homes with needs such as showering, dressing, home help, shopping, food preparation, and cleaning.
  • Kaumātua Day Programmes - day programme in Taupō and Tūrangi that includes activities such as Tai Chi, line dancing, upright exercises, swimming, gentle exercises and crafts.
  • Long Term Conditions Nursing Service - for people with conditions such as diabetes, heart and respiratory conditions. Aim to encourage clients to take a more active role in the management of their health and provide information and skills to give the client confidence to self-manage.

  • Te Aka Matua Health Service - our kaiwhakahaere works with whānau to help to prevent repeat admissions to hospital. We will provide you and your whānau with the information and support to maintain your health in partnership with you and others involved in your care such as long-term conditions nurses, your doctor and others. Our kaiwhakahaere is also able to give cultural support to you and your whānau if you are admitted into the hospital.
