Older patients (65 plus) received their annual flu immunisations

Flu immunisation target: 75% of enrolled patients aged 65 years and over receive their annual flu immunisation. Practice performance on this target is based on annual data. Please note that practice performance against this target may be lower than expected as many people do not receive their flu vaccination at their general practice. They may get it while at work, at their local pharmacy or in an aged care facility.

Before School Check referrals for weight management are acknowledged by the practice within 30 days

Raising healthy children target: 95% of children who have been identified as obese through the B4 School Check programme and referred to the practice are being provided with care. The practice target for this deliverable is that these referrals need to be accepted within 30 days.  Performance of this target is collected quarterly. Please note referrals are only sent to a practice with parental/caregiver’s consent.

Patients identified at greater risk of heart attack or stroke have been identified

Long-term conditions management target: 90% of eligible patients have had their risk of heart disease and stroke recorded in the last five years.  Eligible patients are as follows:

  • Males of Maori, Pacific  or South Asian ethnicity aged 35-74 years
  • Females of Maori, Pacific or South Asian ethnicity aged 45-74 years
  • Males of any other ethnicity aged 45-74 years
  • Females of any other ethnicity aged 55-74 years
  • They must also be enrolled with the practice

The performance of this target is based on quarterly data.
