Description
Papatoetoe Family Doctors
We are an independently owned, family-orientated general practice located in the heart of old Papatoetoe. We belong to the Procare PHO and are an RNZCGP Cornerstone accredited practice. Our team of doctors and nurses are passionate about providing quality healthcare that is affordable and tailored to each individual's health needs.
We generally prefer to see patients by appointment, but we can also accommodate urgent cases. Our facility has an on-site physiotherapy clinic managed by TBI Health, along with a psychologist from Auckland Cancer Society, a health coach, and a health improvement practitioner from Te Tumu Waiora
Staff
Our clinic is supported by a Clinical Pharmacist, Here Toitu, Health Coaches and Health Improvement Practitioners.
Clinical Pharmacist
The Clinical Pharmacist is a healthcare professional who specializes in optimizing medication therapy to improve patient outcomes. They work closely with healthcare teams in our clinic to manage medications, educate patients, and monitor progress. The pharmacist collaborates with the Health Coach and HIP to ensure proper medication use and adjusts treatments as needed based on patient responses. At Papatoetoe Family Doctor, our clinical pharmacist oversees continuous blood pressure and glucose monitoring, among other duties, to help maintain and improve patients' health.
Here Toitu
Here Toitu is a support service offered by ProCare and MSD to help you recognise your potential, improve your well-being, and secure meaningful employment. The team collaborates with patients to help them establish goals and aspirations, whether related to health, well-being, learning, earning, or employment opportunities. One of the services offered by Here Toitu is Responding Early, which is aimed at those at risk of losing their jobs due to physical or mental health conditions.
Health Coach
The Health Coach provides continuity of care along with emotional support and guidance. Coaches are integral members of the general practice team, assisting patients in navigating other services and gaining access to any social and cultural support they may need.
Health Improvement Practitioner
HIPs are qualified and experienced mental health professionals who have completed specialist training in the Te Tumu Waioira primary mental health model. The HIP is part of the general practice team and sees people of all ages and at all stages of their lives and with any type of issue related to behaviour change or mental wellbeing. The HIP will help the patient agree on goals and put in place the necessary support, follow-up, and review to help them achieve those goals.
Ask your GP to refer you to one of these services, as they could help you achieve your health goals.
Different charges apply to casual and non-enrolled patients and for other services. Please check with our receptionist or website at www.papatoetoefamilydoctors.co.nz for more details.
