Weight Loss

Individual education

Please ask your healthcare professional to refer you to the CMH Outpatient Dietitian clinics if you are overweight and also have one or more of the following conditions:

Cardiac (risk > 15%, or existing CVD or heart failure)
Requiring weight loss prior to surgery (not bariatric surgery)
Age <25 years
Severe learning disability
Requires English language interpreter

If you require support for weight loss, healthy eating advice and/or physical activity and DO NOT have one of the conditions list above, please ask your healthcare professional to refer you to Green Prescription for support.
