Referral

To make an appointment to see a midwife you can:

• complete a self referral form at one of the Primary Birthing Units
• complete a self referral form at Māngere Community Health Centre (on level 1, follow sign for Midwives Clinic)
• visit the Birthing and Assessment reception area at Middlemore Hospital.

The information is then passed onto a midwife who works in the area where you live and she will contact you directly within 2 days of  receiving your referral
