Description

Update: Our community told us we need to make some changes. In response, we are improving the way mental health services work with you. We want to assure you we will talk with all service users and their whaanau about how changes might affect their care. If you have any questions please talk to your mental health clinician.

Koropiko Mental Health Services for Older People are skilled in looking after the mental health needs people aged 65 years of age or over.

Our service has two main aims:

  • To improve the health of older people, in partnership with the individual, their family, whaanau and/or carers
  • To provide treatment and support that promotes independence and utilises older peoples’ strengths.

Who Are We?

Our team consists of staff from a wide variety of backgrounds who are skilled in supporting older people, their whaanau and communities. Our team includes:

  • Nurses
  • Psychiatrists
  • Clinical Psychologists
  • Social Workers
  • Occupational Therapists
  • Psycho-Geriatricians
  • Clinical Coordinators

Our team is supported by administration staff and a team manager.

Who Do We See?

We provide mental health services and support to people aged 65 and over. We see people with a range of mental health concerns, as well as medical conditions that affect their emotional wellbeing.

Where Will We See You?

We provide support to older people and their whaanau in a range of locations. Our community services team visit people where they live, be it their own home, family home or other facility.  Some people may be invited to attend doctor clinics in Pukekohe or Waiuku.

We also see people at Ward 35 East, a small ward at Middlemore Hospital for people who need inpatient support. In this ward we can provide special assessment and treatment options. This is a short-stay service, with few people staying for more than 4 weeks.  

How Do We Help?

We offer a range of support and treatments, including:

  • Home based treatment and support
  • Specialised and comprehensive assessments
  • Medications
  • Psychotherapy
  • Liaison with other services
  • Advice & support for carers and/or family/whaanau
  • Admission to Ward 35 East, our assessment & treatment ward if required
  • Education and advocacy in mental health services for older people
  • Help with issues around bereavement, elder abuse, driving risks, residential home placement and legal competency assessments.

We use a team based approach to guide your care. Because our multi-disciplinary team includes staff with a variety of skills and experiences we will often see you in pairs.

Our aim is to improve your quality of life by building on your strengths and addressing any challenges. We work in partnership with you, your family/whanau and a variety of services, to achieve these goals.

If you would like to talk to someone with lived experience of mental health and or addictions, contact our Peer TALK phoneline on 0800 234 432. This phoneline operates between 5pm and Midnight, seven days a week, and will connect you with a trained peer support worker who will listen to your experience and help you access further help if needed.

Referral Expectations

Who Can Refer?

We accept referrals from all parts of the community, including:

  • Health professionals and GPs
  • Community services
  • Aged care facilities
  • People with mental health concerns and their families.

To be referred to adult mental health services you must meet the following criteria:

  • Be an adult aged 65 or older experiencing significant mental health problems which cannot be managed within local community resources. (Note: we are able to support people under 65 year of age if they would benefit from our service.)
  • Live most of the week in the Counties Manukau area
  • Be a New Zealand resident or be eligible for publicly funded healthcare.

Referral Process

  • General Practitioners: Please complete and submit the eReferral form which can be found on Medtech and My Practice.
  • Other Referrers: Please complete the referral form and send it to us via fax at (09) 276 0222. We also accept referrals via phone at (09) 270 9797.
  • Contact the us at (09) 270 9797 if you would like to discuss the referral or ask any questions about our service.

What Happens Next?

  • Referrals are reviewed by our triage team.
  • We may contact you or your whaanau/family to find out more about your situation and how we may help.
  • With your consent we may contact the referrer or other services involved in your care.
  • If your referral is accepted we will contact you with details of the team who will be supporting you.
  • If your referral is not accepted we will suggest alternative services or community supports that will help.
