Referral Process
You are eligible to receive our services if you have the following:
• A psychiatric diagnosis,
• Are referred by a psychiatric clinician,
• Have a current needs assessment,
• And are aged 6 years and above
Description
Kakakura works closely with Southern DHB - Mental Health Needs Assessment and Service Co-ordination team, other community groups, GPs and whānau to identify what supports you require to live a fulfilling life in the community.
