Entry may be by self-referral from:
- the whaanau or caregiver of the young person;
- Integrated Locality Care (ILOC) teams;
- primary care (GP); and
- education providers
Description
Supports include:
- Manaakitanga (Holistic Approach to Whānau Wellbeing)
- Wairuatanga (Whānau Dreams and Aspirations)
- Whanaungatanga (Cultural Identity and Connection to Whānau, Hapu and Iwi)
- Mātauranga (Access to local community resources and supports/services)
- Kaitiakitanga (Whānau Health & Wellbeing)
- Mahi Ngatahi (Real talk about mental health, the impacts & coping strategies)
- Te Ahi Kaa Roa (Housing)
- Rangatiratanga (Whānau-Advocacy and Whānau-determination)
This service is aimed at supporting the whānau of young people aged 0 - 21 years to overcome adversity, flourish and enjoy whānau ora so they are better positioned to awhi their loved ones on their journey to wellbeing.
