Referral Process

Entry to this service is determined by the ICLS Provider and relevant DHB clinical team and is aimed at tangata who would benefit from intensely structure mobile supports.

Description

This mobile service is aimed at supporting people with severe mental illness who have the highest support and recovery needs. Structured support is provided, based on the personalised plans developed in partnership with the Community Living or Intensive Community Living Services, tangata and whānau/family.

Under the guidance of nga pou e waru Manaakitanga, Wairuatanga, Whanaungatanga, Mātauranga, Kaitiakitanga, Mahi Ngatahi, Te Ahi Kaa Roa, Rangatiratanga we aim to support tangata to:

  • achieve their goals in terms of work, friendship or whānau/family connections;
  • improve their ability to complete activities of daily living that increase wellbeing and reduces the reliance on formal mental health services and supports;
  • live full and active lives within the community - thriving not just surviving.
