Referral Process

Entry may be by self-referral from:

  • the young person themselves;
  • the whaanau or caregiver of the young person;
  • Integrated Locality Care (ILOC) teams;
  • primary care (GP); and
  • education providers

Or via a referral from one of the following clinical services:

Description

This service supports pepe, tamariki and rangatahi aged between 0 - 21 years with high support/recovery needs who reside in the Counties Manukau catchment area and would benefit from support in the home and/or community.

Manukura support service will provide individualised and whānau based support to Pepe, Tamariki and Rangatahi who have been identified as having additional social and/or emotional support needs.

Supports will include but are not limited to:

  • Manaakitanga (Holistic Approach to Wellbeing)
  • Wairuatanga (Dreams and Aspirations)
  • Whanaungatanga (Cultural Identity and Connection to Whānau, Hapu and Iwi)
  • Mātauranga (Pre-Vocational Education and Training)
  • Kaitiakitanga (Health & Wellbeing)
  • Mahi Ngatahi (Daily Living & Activities)
  • Te Ahi Kaa Roa (Housing)
  • Rangatiratanga (Self-esteem and Self-determination)
