Referral Process

Entry may be by self-referral from:

  • the whaanau or caregiver of the young person;
  • Integrated Locality Care (ILOC) teams;
  • primary care (GP); and
  • education providers
Description

Supports include:

  • Manaakitanga (Holistic Approach to Whānau Wellbeing)
  • Wairuatanga (Whānau Dreams and Aspirations)
  • Whanaungatanga (Cultural Identity and Connection to Whānau, Hapu and Iwi)
  • Mātauranga (Access to local community resources and supports/services)
  • Kaitiakitanga (Whānau Health & Wellbeing)
  • Mahi Ngatahi (Real talk about mental health, the impacts & coping strategies)
  • Te Ahi Kaa Roa (Housing)
  • Rangatiratanga (Whānau-Advocacy and Whānau-determination)

This service is aimed at supporting the whānau of young people aged 0 - 21 years to overcome adversity, flourish and enjoy whānau ora so they are better positioned to awhi their loved ones on their journey to wellbeing.
