Entry by referral from NASC, mental health inpatient services or through coordination by local needs assessment and coordination services.

Description

This service provides housing and recovery-focused support for people with mental health problems. The service aims to help people move towards more independent living or to retain their level of independence. Clients are supported to actively participate in household chores and management. Clients are supported to use community resources that may improve their independence.

This service provides community-based housing and recovery-focused support services for people aged 18 – 65 years who experience mental health disorders.

The service aims to help people move towards more independent living or to retain their level of independence. Clients are supported to actively participate in household chores and management.

Supports will include but are not limited to:

• Manaakitanga (Holistic Approach to Wellbeing)
• Wairuatanga (Dreams and Aspirations)
• Whanaungatanga (Cultural Identity and Connection to Whānau, Hapu and Iwi)
• Mātauranga (Pre-Vocational Education and Training)
• Kaitiakitanga (Health & Wellbeing)
• Mahi Ngatahi (Daily Living & Activities)
• Te Ahi Kaa Roa (Housing)
• Rangatiratanga (Self-esteem and Self-determination)
