The Service Co-Ordination and High & Complex Needs service provides specialist assessment for persons with high and complex needs, service planning and coordination and liaison support with local, regional and national service providers
A Wellness Plan is a tool to help find out what effect a mental illness has on a person's life and helps identify the abilities, resources, goals, needs and the supports which would be useful to themselves and their family and whanau.
Wellness Plans are done to put appropriate supports in place. Copies are usually held by those people and agencies who support you. We ask you to approve the release of this information to these people/agencies. If they do not have this information, they may not be able to properly support you.
